Privacy Policy

This policy explains what information Prosper Urgent Care, Pllc (“Prosper Urgent Care,” “we” or “us”) collects when you use our website, online scheduling, online check-in and text messages (“our online services”), and how we use and protect it. Your medical information is also covered by our Notice of Privacy Practices under HIPAA. You can ask for a copy at the front desk at any time. If the two ever differ, the Notice of Privacy Practices controls for your health information.

Our registered business name is Prosper Urgent Care, Pllc.

• Contact and identity: name, date of birth, phone number, email, address.
• Health and visit information: reason for visit, allergies, medications, insurance details, photos of your ID or insurance card if you choose to add them, signed consents.
• Payment information: card payments are handled by our payment processor, Square. We do not store full card numbers.
• Technical information: basic logs such as time of visit and device type, used to keep the service secure.

• To check you in, treat you, keep your medical record, bill you and meet legal requirements.
• To contact you about your visit, including phone calls and, only if you opt in, text messages (see below).
• To run and secure our systems.

We do not sell your information. We do not use it for advertising, and we do not share it for other companies’ marketing.

You can sign up for text messages by checking the optional text-message box at check-in or by using our text-message sign-up page. Once you do, we may text the mobile number you gave us about your own visit: a thank-you and follow-up, a secure link to your visit documents or results, a login code for that link, and an occasional request to leave a review. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop, or HELP for help. Consent is not a condition of care.

What we collect for texting: your mobile number, your name if you give it, and a record of when you agreed and the wording you agreed to. How we use it: only to send you the messages you signed up for and to keep proof of your consent. Who it is shared with: only the messaging provider that delivers our texts for us, which may use it only for that purpose.

SMS consent is not shared with third parties or affiliates. We do not sell, rent or share mobile phone numbers or text-messaging opt-in information with third parties or affiliates for their marketing or promotional purposes. Texts never contain diagnoses or test results.

• Other health care providers, labs, pharmacies and insurers, as needed for your treatment and payment.
• Service providers that help us run our systems (secure cloud hosting, payment processing, text delivery). They may use your information only to provide their service to us, and those that handle health information are bound by privacy agreements where the law requires one.
• Government agencies or others when the law requires it.

Our systems use encryption in transit and at rest, individual staff sign-ins with a second verification step, role-based access, and an audit log of who views records. No system is perfectly secure, but we work to protect your information and will notify you of a breach as the law requires.

• See or get a copy of your records, ask for corrections, or ask for limits on how we use them.
• Opt out of texts at any time by replying STOP, or by telling the front desk.
