What to bring
- Your school, team, camp, or employer form (for Florida high school sports, the FHSAA preparticipation form)
- Glasses or contacts, if used
- A list of medications and allergies
- For minors: a parent or guardian
Add-ons
Some teams and employers require extra screening. You can add an ECG for $15 or a TB skin test (PPD) for $35. A TB skin test needs a quick return visit two to three days later to be read.

How much is a sports physical?
School, sports, and work physicals are $45. An ECG is $15 extra and a TB skin test is $35 extra, only if you need them.

Yes. Bring your form and we complete it at the visit.
